This is an independent initiative on Covid19 with the sole aim of triggering a collective contribution from a bottom-up approach while the Government, Frontliners and others are already doing a marvellous job.

It is a social crowdsourcing experiment to collect location based data related to Covid19. The spread and impact of Covid19 would be collected area-wise in terms of your perception towards your health status, vulnerability, exposure and risk.

This survey consists of a maximum of 11 Questions that may be answered in less than 5 minutes without having to respond to all the Questions in certain cases. You are also kindly requested to provide your basic details and location. You may also complete the questionnaire on behalf of somebody else as per his/her details and location. Rest assured, your participation would remain ANONYMOUS. Thank you!

NB: 

(1) Please allow permission to locate your AREA onto a Map or you choose it from a list. Keep the Marker position away from your house.
(2) Questions related to your health status is for self-assessment only and in no case it is to be taken as a medical diagnosis. 
(3) Should you hesitate to share any information, please refrain from this survey.
(4) Upon significant number of responses, the spatially collected data would be curated, aggregated and released through a proper channel to those interested for conducting constructive analysis and research.
